I was told to kill what is under the church, but have no idea what to do next for that particular ending

I've been trying to get An ending after the witch told me to revist locations and the only one I've been able to do anything about was the ice gate in the water temple. I don't know if I missed anything or what but I can't complete the game and it's making me sad hahah....

As the tittle says, there is a achievement on finding someone in a dungeon, problem is, i have no idea what to do or HOW to do.

I'm having serious trouble unlocking the achievement.
