Only got All puzzles achievement, one puzzle and 5 mins are broken. Obviously, if I did all puzzles and spent time doing them, other two should also be unlocked.

There's some sort of a Steam duplicate item error whenever trying to purchase your bundle. With just the bundle in he cart and attempting to purchase, there is a duplicate item error and it fails to allow the purchase to go through. I contacted Steam support and it appears they don't have a solution on their end; they claim to have forwarded off my ticket to the "appropriate department" but at the same time recommended I contact you as well....
