If your analytical methods are indeed flawless, I have only one guess: the mixed partials of your scalar function f are not equal at the point of differentiation.

Do you have any reason to believe those partial derivatives might not be continuous? It appears to me (looking at (9)) that your partials are continuous everywhere except at the constant vectors r and t. Do r or t happen to be critical points of your scalar function? In your calculations you assume the derivatives of f are zero. If r or t are critical points then you're computing a derivative at a point where the second derivative isn't continuous, so you can't assume symmetry of derivative-taking as you do in (6).

Other than that I'm not sure but at the very least here's a bump.
