As a biomedical engineer: Great simulator for starting but I can understand that people can be confused with controls. Its because in laparoscopy instruments are not controlled like using regular thumbstick controller (http://www.laparoscopic-trainer.com/home-laparoscopic-trainers)
That being said, it is nice simulator but needs a little bit of work in tutorial itself to be less confusing to nonmedical realted people.

Great game, 100% worth buying!
I suggest you to find better controler configuration on SteamVR because vanilla ones are made for HTC.
