Everything is zone based now, so you'll feel like you're just starting over every time you enter a new biome, you'll run and talk to people to find the same things you found 2 hrs ago and you'll need new gear. Mob rarity is based on mob type with predictable spawn areas, with some additional repeating 'named' mobs appearing in those packs.
