Ok, go to a single player game, and go to the console. Pick an empty key, that doesn't have any use in your game. I use g for scream, but you can use any open key. So, In the console, type:

bind "g" "vocalize playerdeath"

Then when you are playing, just hit the key, g, and you will scream. There's a bunch of different things you can bind.

This lists all of the commands you can say. Just pick another key, and then replace the vocalize playerdeath, with whatever you want.
