I do, always play vanila.
I usually play vannilla but I have found some interesing mods. I will try polyamorous mod and busty mod :D
Mods can mostly be divided between small improvements or big overhauls (plus expanding aspects like additional / re-balanced classes, weapons, challenge maps and such).

That's the one advantage in terms of unofficial content the Pc (this) version has over the Console versions is expanding the game once the player is comfortable with vanilla.

Yes, I love the vanilla game. I spent many years only playing vanilla. However, it was a great awakening when I finally decided to try some mods. I switch between different batches of mods and vanilla frequently.

Some mods can prevent multiplayer feature, but otherwise mods are fun and add variety, especially those endless/random dungeon mods and pet mods.

Vanilla means playing the same campaign, but with endless/random dungeon mods, you can treat this game like a roguelite by trying new maps from the Map vendors and mod-added special dungeon portals.
I play totally unmodded when I play.
I play with only one mod, and that's to keep the engineer summons between screen changes. Apart from that I play Vanilla.
